Other Forms
If you are requesting patient records, please use the following forms:
- Patient Wizard – This option is exclusively for patients seeking a copy of their medical records. It allows patients to complete the form online, thereby avoiding a trip to the practice location.
- Request Submission portal – This option is for those requesters that already have a completed request and authorization ready to submit (ex. Attorneys)
