Office Information

Office Hours and Appointments

North Alabama Bone & Joint Clinic is open Monday through Thursday 8am to 5pm and on Fridays 8am to 4pm. Our phone number for appointments and other information is (256) 718-3200. For most appointments, referrals are not necessary. Please plan on arriving 15 minutes prior to your appointment time at your initial visit to allow time for registration. For follow-up visits, we ask that you arrive at, but not prior to, your appointment time so that we may stay on schedule, barring any unforeseen emergencies. If you are unable to keep your scheduled appointment, please notify us as soon as possible.

Billing and Registration

Upon your first visit you will need to complete a registration form. Please be prepared to present your insurance and other billing information. We are committed to providing our patients with the best possible care and are ready to assist you with filing insurance claims to insure that you receive the benefits entitled to you. However, all services are provided on a fee-for-service basis. Payments for office visits, insurance co-payments and deductibles are due at the visit when services are rendered. We accept cash, personal checks, MasterCard, Visa and Discover.

Insurance

Your insurance coverage is a contract between you and your insurance company. As a common courtesy, we will file your medical charges with your insurance carrier. Please understand that Federal law prohibits us from waiving deductible or co-insurance payments. Regardless of insurance coverage, you are ultimately responsible for your health care bill. All charges are due and payable on a monthly basis. If payment cannot be made in full, our Patient Accounts Representative (256) 246-3235 will be happy to assist you to set up payment arrangements.

Injured on the Job?

If you were injured on the job, please bring a written statement from your employer authorizing treatment and indicating that the employer is responsible for payment of services rendered. Your treatment will be delayed if this information is not available when you present to our office.
